Warm Pacific greetings to each one of you! As per the subject of this email, I write to request each Full Member to confirm in writing to me no later than Monday, 12 November 2018 the name of your delegate to the 2019 PDF General Forum to be held at the Tanoa International Hotel, Nadi, Fiji on Monday, 25 February 2019. According to the PDF Constitution Clause 9.8, "Full members shall be represented in the General Forum by an individual who is:

a) An office bearer or the executive officer of his or her organisation; and

b) A person with a disability."

In previous General Forum, member DPOs are represented by their Chairperson/President, or Board Member if the President/Chairperson is unable to attend, or their most senior employee (General Manager/CEO/National Coordinator/etc) if neither the President/Chairperson or Board Member are not available. The other key criteria is that your delegate must be a person with disability.

Kindly note that travel, hotel accommodation, meals and other approved expenses for all delegates  and their personal assistants will be covered by PDF. All delegates will also attend other meetings organised by PDF as part of the 6th Pacific Regional Conference on Disability being held at the Tanoa International Hotel, Nadi, Fiji from 25 February 2019 to 01 March 2019.

When confirming your delegate, please note the following:

1.       Include the full name of your delegate as in his/her passport.

2.       Include the full name of the delegate's personal assistant )PA) as in his/her passport only when your delegate needs a PA.

3.       The passport of your delegate and his/her PA must be valid for more than 6 months from February 2019.

4.       Send the passport biodata or photo page of your delegate and PA where applicable.

5.       City/Town where your delegate and his/her PA reside as this is useful when travel arrangements are being organised.

6.       Your delegates' email address and telephone contacts.

Upon receipt of your delegate's name and details, the conference logistics team will contact him/her for registration and other purposes. I look forward to your timely and positive response on this matter.
